Key points

  • The effect on LDL cholesterol builds over about 2 to 4 weeks.
  • Treatment is intended to continue long-term.
  • It is not recommended for people under 18 years of age.

What it is and what it is used for

Bempedoic Acid is an ATP citrate lyase inhibitor and belongs to the lipid-modifying therapeutic class. It is used for adults who have primary hypercholesterolaemia or mixed dyslipidaemia and need further LDL cholesterol reduction. If cholesterol remains above target, it can be added to a statin or another lipid-lowering medicine. It may also be used alone by people who cannot tolerate statins. The aim is additional LDL reduction when existing treatment leaves cholesterol above target.

How it works

Bempedoic Acid inhibits ATP citrate lyase, an enzyme involved in cholesterol synthesis in the liver. Reducing cholesterol production increases LDL receptor activity, so more LDL cholesterol is cleared from the blood. It acts earlier in the cholesterol production pathway than statins. The medicine is activated in the liver rather than in muscle tissue, which may account for the lower rate of muscle-related reactions compared with statins.

Product form and delivery

The Bempedoic Acid product is a 180 mg film-coated tablet. It is a generic bempedoic acid tablet for hypercholesterolaemia and dyslipidaemia. The tablet is taken once daily. Pharmacy partners in India ship to Malaysia in discreet packaging.

Who should not take it

Bempedoic Acid is contraindicated in people with hypersensitivity to bempedoic acid and must not be used during pregnancy or breastfeeding. Breastfeeding is contraindicated because of the risk to the infant. Uric acid levels should be monitored, and caution is needed in people with a history of tendon disorders. Gout, liver disease or previous tendon problems should be raised before treatment starts. Kidney function and current lipid-lowering treatment should also be checked.

Side effects and interactions

Minor side effects include muscle spasms, back pain and hyperuricaemia. Moderate side effects include elevated liver enzymes and anaemia. Tendon rupture is a serious side effect that requires immediate medical attention. A rise in uric acid may trigger gout. Bempedoic Acid interacts with simvastatin, pravastatin and atorvastatin. When it is combined with these statins, cholesterol response and tolerability need medical supervision. Mild effects are common, but unusual symptoms should still be reported to a healthcare professional.
