Tizanidine

Tizanidine is a centrally acting muscle relaxant taken as an oral tablet. It is used to lower raised muscle tone and ease the spasms and stiffness that accompany certain neurological conditions. It acts on the spinal cord rather than directly on muscle fibres, so tightness settles without a matching loss of muscle strength.

Key points

  • Tizanidine is an alpha-2 adrenergic agonist. It increases presynaptic inhibition of motor neurons, calming overactive muscle signalling while voluntary strength is largely preserved.
  • It should not be used by people with severe hepatic impairment or by anyone taking a strong CYP1A2 inhibitor.
  • Blood pressure needs monitoring during treatment, and elderly patients and children are treated with caution.

What tizanidine is used for

Spasticity appears as muscle tightness, cramping and involuntary spasms, and it can follow multiple sclerosis or a spinal injury. Tizanidine is taken to reduce how often these spasms occur and how severe they are, which makes movement and rest more comfortable. It also has a place in broader pain management where persistent tightness adds to aches and limits range of motion. The medicine eases the tightness a condition produces; it does not treat the condition itself. More detail on the underlying problem sits on the muscle spasticity page.

How it works

The medicine stimulates alpha-2 receptors within the central nervous system. This increases presynaptic inhibition of motor neurons and lowers the excitability of the nerve cells that serve the muscles. Tone falls, yet the nerves responsible for conscious movement are not significantly affected. A dose usually begins to act within 1 to 2 hours.

Forms and brands

Tizanidine comes as a tablet in 2 mg and 4 mg strengths. Zanaflex is the tablet listed here, with tizanidine as its active ingredient. Brand names vary by region while the active compound stays the same. Pharmacy partners in India ship orders to Malaysia in discreet packaging.

Safety and interactions

Tizanidine must not be used by anyone with a hypersensitivity to it, with severe hepatic impairment, or alongside a strong CYP1A2 inhibitor. Ciprofloxacin, fluvoxamine and alcohol are the interactions to watch, and alcohol adds to the drowsiness the medicine can cause.

Drowsiness, dizziness, dry mouth and fatigue are common when treatment begins. Hypotension, bradycardia and elevated liver enzymes are moderate effects that need review. Hallucinations, severe hypotension and hepatitis are serious and require immediate medical attention. Tizanidine can impair the ability to drive or operate machinery, and blood pressure should be monitored throughout. Elderly patients are treated with caution, and children with caution as well. In pregnancy a specialist should be consulted; during breastfeeding the medicine is used with caution.

What the sources say

  • A dose works for roughly 3 to 6 hours, so it can be timed for the moments when relief matters most, such as bedtime (MS Society).
  • Some people become drowsy, and driving or using machinery should wait until they know how the medicine affects them (MedlinePlus).
  • Dizziness, lightheadedness and fainting after standing up quickly are more likely in the first days of treatment (MedlinePlus).
  • Stopping suddenly can bring a faster heartbeat, raised blood pressure and muscle tightness, so the dose is reduced gradually under medical supervision (MedlinePlus).
