Clarithromycin is a macrolide antibiotic, supplied as film-coated tablets in 250 mg and 500 mg strengths. It is used for bacterial infections such as throat and sinus infections and flare-ups of chronic bronchitis, and as part of combination courses that clear Helicobacter pylori from the stomach. The originator company is Abbott, and the tablets are also known as Klaricid. The active ingredient is clarithromycin.

Clarithromycin binds to the bacterial 50S ribosomal subunit, blocking protein synthesis. It strongly inhibits the liver enzyme CYP3A4, and serious reactions have been reported when it is taken with other medicines broken down by that enzyme. It must not be combined with astemizole, cisapride, domperidone, pimozide or terfenadine, because this can cause QT prolongation and dangerous heart rhythms; deaths have occurred from interactions of this kind. A history of QT prolongation or severe liver problems rules it out.
