The tablets act on the hypothalamic-pituitary axis, using negative feedback to suppress gonadotrophin release and inhibit ovulation. Combined oral contraceptives raise the risk of venous thromboembolic events such as deep vein thrombosis and pulmonary embolism, and smoking adds to the risk of serious cardiovascular events. Levlen is contraindicated in women over 35 who smoke, and in women with a history of venous thromboembolism, ischaemic heart disease, migraine with aura, or diabetes with vascular complications. It is contraindicated in pregnancy and should be avoided during breastfeeding.
